5. Daisys Third Eye
A bizarre and strangely disturbing bug can be found on the Princess Daisy trophy in early copies of Super Smash Bros. Melee. If you zoom in through the hair on the back of her head, you will find a third eye staring back at you.
It is a complete mystery as to why or how this eye made it on to Daisys trophy. The art style doesnt resemble that of Daisys eyes. Its likely that it was a template or placeholder left over and forgotten about, or maybe somebody placed it there intentionally as a joke.
After the third eye was spotted and word of its existence spread, it was removed in later versions of the game.
