3. Evil Otto From Berzerk Causes Death

Don't Look into his Eyes! You think Polybius' addiction and night terrors was scary enough? Well, an arcade game made by Atari has actually caused two teenagers to suffer heart attacks, and die as a result. This game was called 'Berzerk,' and the tragic story was linked to the game's 'boss' known as 'Otto' who is a pixellated red smiley face. The game was a simple early-era puzzle game where the player would navigate a yellow robot-like figure through a maze without touching the walls or bumping into enemies. Otto's job was to zap a player if his or her time was running out. The game had a mind numbing 64,000 levels, and what both of Otto's victims had in common were ridiculously high scores. One of the duo suffered his heart attack after besting his score yet again, and the other beat his game in 15 minutes a year later before shuffling off the mortal coil. The gamers were named Jeff Dailey and Peter Burkowski, and to date, it is believed this is the only game reported to actually cause death.
