5. Tiny Tina's Eyes - Borderlands 2

First introduced in Borderlands 2, Tiny Tina is an absolute nutcase with a penchant for explosives and a deep love of crumpets, and will assist the Vault Hunter in their mission to stop Handsome Jack.

After encountering her for the first time, players began to notice that her left eye would occasionally flit back-and-forth (while her right eye remained still), which, although it fit well within the wacky world of Pandora, looked like a straight-up error, and many players assumed that it wasn't supposed to happen.

And that assumption was true. Partly. While developers Gearbox Software never meant for this to happen while they were designing and coding Tina, they decided that it matched her utterly insane and bonkers personality, and chose not to fix it.
