Without A Trace is a funny quest since you’ll be given a contract for it from a local Herbalist but you don’t actually complete it properly without doing some proper investigating which will trigger the quest’s true ending.

Picking up the contact off a local notice board is absolutely standard fare for Witcher 3 side quests but things get a little messy from there on out. Otto, the Herbalist, tells you his apprentice has gone missing. Your investigation takes you to a small, almost abandoned village where you discover no trace of him and an old couple tells you they’re pretty sure the apprentice was attacked by wolves. Here your quest log goes "Sick, case closed!", and tells you to go back and give the Herbalist the bad news. You can do just that or you can trigger the next stage of the quest by noticing the village is covered in blood, and that one gate of the town has a stink trail of rotten meat for Geralt to follow.

I personally stumbled on that one on my way out and finding out there’s more to it is both satisfying and deeply unsettling as it becomes apparent the old couple have chopped up the apprentice for food.

There’s no way to know if they killed him or if it was wolves as they continue to insist, but you can either let them go on their cannibal-y way or kill them off to end the quest.
