We can't stress this enough:
Save. Often.
The number of times a perfect run can be derailed completely by a mistimed move from the shadows, or even worse a guard rounding the corner and witnessing you filet a man with an antique sabre is much higher than Agent 47 would probably care to admit.
The best thing about the Hitman games is that the AI has patterns that can be learned but in the heat of the moment all of this can be forgotten, therefore it always pays to save after important moments. The game offers a decent checkpoint system, but my advice would be to save after every kill or just before you approach your assassination targets.
Things can go south quickly, and so being able to reload a save only a few minutes beforehand makes the world of difference, plus it eliminates and backtracking you might need to go through if you rely solely on checkpoints. Plus if you're a real sadist you can reload these saves over and over and try out different murder methods.
