It's supposed to be used to knock guards unconscious from afar (and you can eventually unlock an explosive variant to boot), but more hilariously just bank a hard left or right and re-direct it back towards Big Boss's Big Boss. Connecting anywhere on him throws you into the 'imminent death' animation where the screen goes red and you have to mash the buttons to keep him alive. Forget 'phantom' pain, this is as real as it gets.
