When dreams become reality, holograms will cheer. Such is the situation here as the Emergency Medical Hologram received one of the biggest jumps in rank of any character in the franchise. That is to say - he went from having no rank at all to becoming the commander of Voyager, when the situation arose.

This saw the EMH switch from his usual blue medical uniform to a fully-pipped command red - although the first time that this happened, it was but a daydream, concocted by a very bored hologram.

The Hierarchy, an alien race who would attempt to spy on the ship, accidentally tapped into the EMH’s dreams of a more exciting life - believing him to be the dangerous tactical commander he imagined himself to be.

While that would end up saving the ship, it also gave Captain Janeway the idea to actually create an ECH - that is to say, when the ship was robbed of its crew in the two-parter Workforce, Janeway was able to transfer control to the ECH before things became too dire. Though life may have begun as a dream, seeing those four pips blossom into life would send shivers down all audiences’ spines.
