Death Toll: Over 50 Realities

The Anti-Monitor is one of the DC Universe's greatest villains. He was the principal antagonist during the Crisis on Infinite Earths crossover event that reshaped DC Comics from the reality it operated in from its inception to 1985.

The Anti-Monitor hailed from the Qwardian universe, which he conquered. Looking to conquer more than just his own universe, he set his sights on the many worlds of the Multiverse, which brought him to the attention of pretty much everybody in the positive matter universes who combined their forces to challenge the Anti-Monitor.

Many lives were lost in battle including Supergirl and the Flash. Ultimately, the Multiverse was destroyed leaving only a single reality and a new Antimatter Universe left for the survivors. Many of them were saved and taken into a Paradise Dimension, but the ones who weren't were erased.

The event reshaped the DC Universe and cleared up a lot of inconsistencies from years of multiple realities. That being said, the Anti-Monitor's destruction of the entire Multiverse means he was responsible for the deaths of an infinite number of realities and every being who lived within them. Body counts literally cannot get any higher than that.
