Death Toll: An Entire Reality

Superboy-Prime hails from the reality known as Earth-Prime, which was devoid of all superheroes, but featured them in comic books. Basically, it was our reality, only there really was a young man named Clark Kent who turned out to be a real-life Superman just as he was depicted in the comics.

This realization made the young man believe he had a certain destiny to reach, which led him to become a superhero. Unfortunately, Crisis on Infinite Earths left his reality erased, and because he believed himself to be the one true Superman and his reality the only proper one, his morality became somewhat warped.

He soon became an incredibly dangerous supervillain who believed that all realities but his own original one were corrupt. This led him to become a psychotic killer whose power caused massive destruction across multiple realities.

While under the moniker of Superman-Prime, he battled Monarch and the heroes of Earth-51. When Prime ripped open a part of Monarch's chestplate, a massive release of quantum energy resulted in the complete destruction of Earth-51. That puts Prime's body count at an entire reality of everything that ever was or would be. You can't count numbers that high...
