3. Ra's Al Ghul
Not that Bane was the only one pulling strings that whole time. On more than one occasion he has allied himself with the League of Assassins, putting him directly beneath Ra's Al Ghul. The Joker might be Batman's most famous and feared enemy, but Ra's is amongst his most feared and powerful.
Ra's Al Ghul might not be as physically strong as Bane, have the insane level of power that Poison Ivy wields, or anything of that sort. What he does have is a genius-level intellect, some increased strength and stamina and immortality from frequently stewing in the Lazarus Pits which bring the dead back to life. Which admittedly also made him a bit loopy, but the dude's managed to life for over 600 years and retained all the knowledge and skills he's accumulated in that time.
More than any other villain, he's come closest to not only defeating Batman but destabilising the world. He's not just powerful: he's capable of affecting the entire Earth.
