2. The Hulk
The Punisher has managed to kill the Hulk a few times across various alternate realities. In The Punisher #37, which saw the Punisher locate Bruce Banner in a sickly state. He took him in and nursed him back to health, though he was afflicted with amnesia.
While feeding him, he threw in some explosive material along with some supplements to impede digestion. Once he was fully laced with the right amount of Boom, Castle forced a transformation into the Hulk and pitted him against Wolverine, Spider-Man, and Daredevil. When the Hulk beat everyone, Castle detonated the explosives, ripping the Hulk apart from the inside, and forcing a transformation back into a very deceased Banner.
In Marvel Universe vs. The Punisher, the virus that turned everyone into horrific cannibals infected the Hulk, and it was up to the Punisher to put him down. It's no easy task, as this is the Hulk, and he has an army of followers.
Eventually, they faced off against one another, and the Punisher managed to kill the Hulk with a bow and arrow. Granted, this wasn't a typical arrow, as it was made from one of Wolverine's Adamantium claws, which made it possible to shoot through the Hulk's eye and into his brain, killing him instantly.
