Interestingly, one of the grossest and absolutely worst things to have happened to the Joker is also one of the most recent, despite his role as DC's biggest villain currently being borrowed by the Batman Who Laughs.

After discovering that this chuckling version of the Dark Knight was created when Batman killed the Joker and was exposed to the toxin that released from his upon his death, Lex Luthor comes up with a plan to stop the universe's current threat. In order to do so, he has to poke around in the jester's brain in order to figure out what exact chemicals are concealed within him.

Though Lex appears to do this through his recently acquired telepathy, the experience appears to the Joker like he is having brain surgery done on him. While conscious - which is normal for brain surgery - but also notably without anaesthetic or any kind of pain killer, which isn't normal whatsoever. Though the Joker doesn't react facially, it's clearly an unpleasant experience for him, as when Lex finishes rifling through his mind, the clown has collapsed on the floor.
