A similar fate befell the cover of this gonzo X-Men spin-off that introduced the world to a gay werewolf, a bisexual bug man whose power came from his sweat glands, and Dead Girl (whose mutant power was that she was dead).

That all passed by the usual Marvel Comics standards and practices fine, it's just when they tried to add a superheroic, resurrected Princess Diana to the team they objected. Michael Allred still drew the cover, though, so somebody signed off on it at some point.
