How Did He Die?

The dashing "French" rogue with a penchant for murdering child versions of Apocalypse died a hero, saving Psylocke's life and removing her from the clutches of the Shadow King. In doing so he was unable to escape his old colleague/nemesis from the Weapon Plus program, The Skinless Man, who tore out his heart. Ouch.

How Did He Come Back?

Fantomex was cloned back to good health... almost. As a survivor of the Weapon Plus program, Fantomex had three brains thanks to all the tinkering done on his physiology by the Weapon Plus program. When he was cloned, each of the three brains was given a body somehow, because, comics. So there's now the original Fantomex as we know him, a female Fantomex called Cluster, and a third, more nefarious individual who calls himself Weapon XIII.
