How Did He Die?

Wolverine went on a brainwashed killing spree one time thanks to an alliance between Hydra and The Hand. Northstar tried to get in his way and it just didn't end too well for him.

How Did He Come Back?

Hydra and The Hand ended up absconding with Northstar's body and resurrecting/brainwashing him in the same fashion they had done Wolverine. It took some time before he was able to break free of his mental conditioning, but he got there in the end and is an active X-Man once more.
