How Did He Die?

Attempting to stop a psycho Xavier Security Enforcer from the future named Greystone, who had designed a sort of time machine/bomb that was dangerously unstable. Havok stopped Greystone, but the bomb went off. His on-again/off-again girlfriend Polaris could only watch as Havok was seemingly disintegrated.

How Did He Come Back?

Havok was actually catapulted by the explosion into an alternate timeline (I mean, sure), where he became Mutant X, the leader of a strange world's X-Men.

In the Mutant X timeline he discovered his body was the nexus point for all the Alex Summers' across all dimensions, and naturally this led to a moment where he saved the multiverse ("naturally"). Poor old Alex "died" again in doing so, and was restored to his consciousness back in his own world.

His comatose body was found in some random hospital, where a small psychic boy slowly helped him come to.
