How Did He Die? Went a little crazy, after he found out his ex-girlfriend Storm was getting hitched to Black Panther. Got hit on the head by Bishop one time. After that, he kind of lost his marbles, and started experimenting on humans to turn them into a new mutant race. He learned of an imminent attack from a parallel dimension, which the X-Men were also tracking.

Using a series of devices called 'Ghost Boxes', he suggested to his old teammates that they go to the parallel dimension and fight the enemy head-on, sacrificing their lives. The X-Men obviously weren't too fond of that idea, but Forge had kind-of, maybe, sort've turned on the Ghost Boxes already - which of course would have let the enemy aliens come through to Earth as much as the other way round.

S.W.O.R.D. Agent Abigail Brand fires a rocket at Forge's base from the sky - that's one way to close a dimensional breach, after all - and Storm implores him to leave. He refuses. Kaboom.

How Did He Come Back?

Forge just kind of appeared to be alive and well, with the implication that he ultimately didn't take getting blown up lying down. Cable finds him (after returning from his own death) and helps Forge rid himself of insanity, in exchange for some sweet upgrades.
