While it may be an odd thing to consider, there could be a case made for Brandon Lee's death during the filming of 1994's The Crow being the most infamous death on a film set ever.

Brandon was the son of famed martial-artist and film star Bruce Lee and had completed most of the filming when he lost his life due to a tragic mix up that involved a firearm. The specifics of it are a little hard to put together for any without knowledge of guns, but this is the basic overview of what happened.

A firearm had been gimmicked to fire a dummy round, which is when a bullet is placed in the barrel and used for shots where the camera wants to see that a gun is loaded. The bullet became lodged and the shoot moved on to a scene that required a blank - when there is gunpowder but no bullet, used when the noise and muzzle flash are needed.

No one ensured that the barrel was clear so when the trigger was pulled, the bullet fired, piercing Brandon in his abdomen and killing him.

Filming was completed using camera tricks, previous shots and stand-ins.
