1. Choosing To 'Rescue' Sirius

Harry has made some questionable decisions in the past but falling for Voldemort's deception in Order of the Phoenix had one of the worst results. Through his connection with Voldemort, Harry saw a vision of his godfather, Sirius Black, being attacked at the Ministry of Magic - but it was a false vision.

Taking some of Dumbledore's Army with him, Harry charged head first into a trap set by the Death Eaters. The Order were forced to save them, which resulted in Sirius Black being murdered by Bellatrix Lestrange.

Putting himself and his friends in danger, Harry made a stupid choice and fell for Voldemort's bait. It resulted in Sirius dying and the Death Eaters very nearly escaping with the prophecy. It was one of Harry's worst decisions, and it killed the only family he had left.
