7. Sadistic Fallout: New Vegas Realism Mods Force Players To Monitor Everything

Fallout New Vegas The modding communities for Bethesda RPGs in general are freaking insane, and Fallout: New Vegas seemed to attract the most sadistic brand of modders, who essentially wanted to make the game as punishingly difficult as possible, to the point that just surviving a day seems like a chore, and is probably more difficult than it would be in a real nuclear apocalypse. The game itself already has a Hardcore mode, in which players have to eat, drink, sleep and tend to their wounds responsibly, but the modding community has taken to this to an utterly mind-boggling level. Some of the most popular aspects of the various "realism" mods include one-shot kills for both the player and enemies, heavier items, a nutrition manager that requires players to ensure they eat enough protein and so on, and even following the right sleep pattern. Oh, and eating or drinking too much can result in death or debilitating side effects. Hooray.
