9. Richard Trager (Outlast)
Although there are plenty of creepy characters that stalk you through the abandoned Mount Massive mental asylum in Outlast, none are quite as threatening as the sinister Doctor Trager. Upon capturing you and tying you to a chair, the good doctor lets you know he means business immediately by slicing off one of your fingers. Getting up close in personal, Trager is perhaps the most human of the enemies in Outlast, yet somehow that only makes the horror more realistic. Once you escape your trap it doesn't take long for him to know you're missing, and he'll chase you around the claustrophobic wing until you can get your bearings and escape. Unlike the other enemies in the game, this psychopathic surgeon is faster and more agile than your regular demented nutcase, and as a result the escape is much more intense than any of the other set-pieces in the title. While the music wails and the screams of his other patients are heard in the distance, if Outlast doesn't get your heart pounding at this point, it probably never will.
