Mytha, the Baneful Queen is not a bad boss fight - unlike a few entries on this list there was no sort of huge outcry as to her inclusion, her quality or her mechanics. The gimmick concerning her arena, however, was an odd combination of obscure and illogical.

If a player fought through the level as normal and entered the boss fog, they would face the Queen in a room where the ground was full of poison. This otherwise fair fight would become a balancing act between healing, curing your poisoned status, and actually fighting the boss in front of you.

If, for reasons better known to themselves, a player chose to light a torch at a bonfire earlier in the level and walk up to the spindle of an otherwise unmarked and unremarkable windmill, there would be a command prompt to set fire to it. Once it was alight, the boss arena would be changed, with most of the poison removed and solid ground to fight on.

Why this in-game function that was never hinted at and not repeated was included here and here alone is a truly baffling design choice. And how anyone found it by accident is equally confusing.
