Silent Hill: Homecoming featured scenes so violent that it was initially banned for sale in Australia and Germany, with developers Konami needing to make changes before the countries' respective ratings boards would allow the game to hit store shelves.

One of the nastiest scenes in the game came when the protagonist, Alex Shepherd, was captured and taken to the secret cult's lair. He wakes up strapped to a chair, with the evil Judge Holloway looking over him and revealing some of her own dark secrets. She picks up an electric drill and approaches Alex, who tries to talk her down. But she's not having any of it.

She fires up the drill and jams it into Alex's leg. His entire body convulses horrifically as she pulls and pushes the drill back and forth into his thigh, and players were left cringing on the sidelines as they watched the scene unfold. Eventually, Alex manages to break free, pushing Holloway back. She attempts to force the drill into his eye, but he eventually overwhelms her, using the drill to pierce her jaw and thrusting it up into her head.
