9. Mass Effect 3 - Tali
If you're a Mass Effect fan, there's a chance you'll have seen the title of this entry and thought "Wait, Tali can die in ME3?!" If so, congratulations! You belong to the elite group that didn't make a total hash of resolving her storyline.
What makes this particular death sting so hard is that it is almost entirely the fault of the player. Tali's swan dive off a cliff only occurs if you make so many poor decisions during the Rannoch story arc in ME3 that Tali is forced to watch her entire race get obliterated by their centuries-old nemesis.
So, no pressure or anything.
In a final, diabolical touch, Bioware trick the player into thinking Tali can be saved. As she falls to her doom, Commander Shephard (the player character) leaps to save her just as a QTE flashes on screen. If you react in time, Shephard desparately stretches out their hand and grabs... nothing. Nothing but empty air.
God damn it, Bioware. Not like this. Not like this.
