7. The "True" Ending - Bloodborne

FromSoftware games are challenging enough without having to consider alternate endings, and unlocking Bloodborne's third and "true" ending requires an even greater heft of effort.

Though the first two endings are unlocked easily enough - either submitting to final boss Gehrman or refusing him - the best ending requires the player to obtain three "One Third of Umbilical Cord" items, achieved by undertaking a rather tricky trio of fetch quests.

Then before meeting Gehman, you have to consume the three items and select "refuse," which triggers the "final" boss fight against Gehrman.

Afterwards, you'll see a new cutscene where an extra final boss will be revealed, the Moon Presence, which is mercifully one of the easiest boss fights in the entire game, followed by an extended ending cutscene.

Bloodborne is plenty tough without putting yourself further through the wringer for a few extra minutes of content, especially given that most players needed a guide to even figure out how to unlock it.
