5. Good Listener & Pit Boss - Portal 2
Portal 2 gets the distinction of granting the player two achievements for getting themselves killed, and they're both relatively tongue-in-cheek rewards that make fun of the player's interactions with the kooky personality cores GLaDOS and Wheatley.
First up we have Good Listener, which is obtained by "taking GLaDOS' escape advice." During Test Chamber 21 in the game's fourth chapter, GLaDOS will open up a pathway for the player, which she claims is the "final" test.
Follow this path and you'll realise quite what finality she meant, as you'll promptly be gassed to death.
Secondly there's Pit Boss, which similarly has a vague objective description - "Show that pit who's boss."
It's unlocked at the very start of Portal 2's ninth chapter. When you teleport away from Wheatley after he attempts to crush you, he'll beg you to come back, and while player instinct says to ignore him, you can get an achievement by defying your innate sense of self-preservation and actually listening to him.
Wheatley then tries his luck by asking the player to jump into the pit in front of them, and if you actually do it, you'll get the Pit Boss 'cheev, all while Wheatley expresses hilarious surprise that his silly gambit actually worked.
