Pretty much all Witcher 3 players agree that the worst ending in the game is Ciri dying while trying to deal with the White Frost. It happens when you act overly protective of Ciri, which is why it can come off as a shock to most players, even though the game does foreshadow what consequences your actions will have.

During the mission "Blood on the Battlefield," there is an interaction between Geralt and Ciri after the girl grows frustrated with her magic training. The player can console her by saying she doesn't have to be perfect, or distract her with humor.

If the player chooses the first option, Ciri will feel less inclined to work on herself, resulting in her death in the final act. The game indicates this with the shot of a cat carrying a dead swallow in its mouth, which may seem unimportant at the moment, if you weren't paying attention.

The shot is clearly symbolic of Ciri's death. Her elven name translated is "swallow" and several characters use the word as a nickname for Ciri.
