1. Hot Coffee - Grand Theft Auto: San Andreas
The Grand Theft Auto series hit its peak during the 6th generation of home consoles. Rockstar's dedication to the series resulted in 3 stellar entries in 4 years from 2001 to 2004, stopping only in 2003 to release the highly controversial Manhunt.
It's not like the developer were deviating too far from their usual development process, however, as GTA has also seen its fair share of controversy over the years.
Grand Theft Auto's constant insistence on pushing the boundaries of fiction has landed Rockstar in the centre of much unwanted criticism. The back of the box is littered with parental warnings and ESRB caveats, so it was of no surprise when PC gamers uncovered a usually inaccessible mod titled 'Hot Coffee', a Rockstar-developed minigame whereby the player controls CJ's, urm, sexual manoeuvres.
The minigame sparked much controversy, and the ESRB promptly began an investigation into matters. In response, Rockstar thought it best to remove the feature, and thus it was locked away forever. That was, until crafty PC players gained access to Hot Coffee, rendering it public knowledge.
