Even despite the horrifying implications of the deeply disturbing, and terribly invasive prostate exam that appears to be happening here, if you look at the intricate details a whole new dimension of dismay is revealed. That "Choke Hold - Need Assistance" offers further insight that the player is apparently attempting to choke hold his opponent from the inside, and you have to think this is the most grossly over the top "distraction" technique anyone has ever attempted in a stealth game.

Frictional Games has a certain knack in creating morbidly disgusting monsters, and to make things even worse, there's virtually no way to defend yourself from any of them. Running isn't an option, either, because the protagonists seem to have their legs beaten up before the game starts. The only thing you can do is to hide and hope for the best, which is clearly not all that successful when faced with imminent attack and certain death.
