There was really no question as to what was gonna be at the top of this list. One of the hardest and most frustrating bosses of all gaming history, mostly due to just how hollow you feel after winning. Sans will run you ragged and the game is specifically designed to make you feel no satisfaction or joy in your victory.

For the few dozen of you who aren't aware, there is only one way to get this boss fight. You gotta kill EVERYONE. As in, go out of your way and methodically hunt down every other person and npc in the underground in order to come to this final confrontation with Sans. There is no way to accidentally fall into this fight. So when you get to this fight, rest assured that you have earned everything coming to you.

Sans is deliberately designed to be the hardest, most frustrating, most unfair boss fight in the history of boss fights, because it's essentially the game punishing you for doing what you've done. To the point where Sans at one point decides to use the turn based nature of the game to say "know what? Nah, you don't get to have a turn anymore".

And the cherry on top? If you've gone this far, then there's also a chance you can permanently screw up your game. Forever. Which makes Sans, without question, the hardest optional boss fight that just isn't worth the trouble. Just don't do it, man.
