1. Unlocking Anita - Marvel Super Heroes

When Marvel Super Heroes came out in October 1995, the first official entry in the Marvel vs Capcom series (X-Men vs. Street Fighter) was roughly two years away from release. Whilst the title was both developed and published by Capcom, the base roster did not feature any original Capcom characters.

But one Capcom character did manage to sneak onto the Japanese retail copies of the game, but can only be unlocked by inputting a cheat code. Anita (from Capcom's Darksiders franchise) actually makes her first playable appearance in Marvel Super Heroes, only serving as a support character within her own titles.

Whilst saying that players were not supposed to find Anita might be a stretch, her inclusion in the game is a somewhat strange considering both how overpowered and clearly unfinished the character is. She has no dedicated victory screen or character portrait either, instead using Thanos' assets in place of these.

Even stranger still, the character cannot be unlocked in either the North American or European copies of the game. Considering the Japanese version of the game was released only three weeks after the American one, this might imply the character was a very late addition to the game.
