4. You Must Kill Yourself To Save The Wasteland - Fallout 3
Everyone points to Mass Effect 3 as the 'one game' that really let down legions upon legions of fans, but like the wealth of bugs also present in Fallout 4 rarely being fully accounted for, Bethesda's past transgressions got swept under the carpet pretty fast too. Looking back, you'll note they actually tried to remove this rather substantial blotch on their record by patching in an alternate ending through paid-for DLC (how'd they get their good reputation again?), allowing the setup of "The radiation will kill you" to be passed over to a number of followers that could always survive such conditions regardless. How to fix it: Bethesda eventually did most of the work themselves, albeit in the most money-grubbing way possible. However, as the entirety of Fallout 3's gameplay is built around player progression and procedural unlocks/abilities, we should've had the ability to extend something like Rad Absorption to the point of withstanding the effects of the area for long enough to get the job done. A similar beat appears again near the close of Fallout 4, where - without delving into spoilers too much - you're tasked with hitting a button in amongst another area that'll kill you in seconds. You somehow pull through thanks to chewing on RadAway and other various meds as your life depends on it - alongside your own abilities - but the option was nowhere to be seen in the original Fallout 3.
