The third trailer for Death Stranding revealed that characters can have "dooms" that are related to the BT's in some way.

What exactly is a BT though? BT stands for Beached Things and they seem to be the big bad of Death Stranding, hunting Sam and the other characters and dragging them into a purgatory. This purgatory resembles a beach as can be seen in Heartman's footage, hence the name, Beached Things.

However, it seems like everyone has a different "doom level" that ties into their ability to view the BT's at all. In the footage of Fragile she asks Sam about his level, where he states that he can sense them whilst she can actually see them.

This has led to speculation that your dooms level increases depending on how much you voidout, slowly making you more stronger but less human as a result. Considering voiding out doesn't actually kill you, this will likely be one of the big gameplay choices you have to make as a player.
