Focussing on Guillermo Del Toro's character, he appears to be some kind of government official. He's wearing a clean suit, and bears a badge of office on his breast pocket.

When zoomed in, the badge reads: "BRIDGES United Cities of America" and shows a spider web pattern spreading out across the country from the East Coast area. This seems to be a hint that the game takes place in the real world, either in the future or in some kind of alternate dimension.

Del Toro's character also bears a scar running across his forehead, which shows a parallel between the two Death Stranding trailers. Both characters have featured surgical scarring of some kind, with Del Toro's looking like it could be a lobotomy scar of sorts. Norman Reedus' character sported a quartered torso scar in the first trailer.
