7. MGS 3's Crazy Dream Sequence Meant More Than You Think

This one's a bit of a two-parter, because on the one hand you may have completely missed the optional mini-game (codenamed 'Guy Savage") where Snake has a nightmare, awaking in a strange sepia-toned hack n' slash game complete with hooks for hands and the urge to slaughter everything around him. To trigger the sequence, you need to have saved your game during the capture/prison sequence (right before he loses his eye) and quit the game (simulating him going to sleep). Rebooting will then drop you in this demo-like dreamscape for a few moments, but the really great part is, the sequence wasn't assembled by Kojima at all. Instead, his close friend and writing whizz Shuyo Murata (a co-writer on every major Metal Gear) actually took the lead on this level, directing it himself. It impressed Kojima so much that he promoted Murata to help co-direct MGS 4 with him in 2008. Not bad for a sequence many of us completely missed first time through anyway.
