2. They Killed George Bush

HBO DEFINITELY don't want you to remember that they rammed the head of a former US President onto a pike alongside Ned's noggin, but here is George Bush in all his glory. A fact that may not have came to light if David Benioff and D.B. Weiss hadn't drawn attention to it during the DVD commentary. The network responded with a statement: "We were deeply dismayed to see this and find it unacceptable, disrespectful and in very bad taste. We made this clear to the executive producers of the series who apologised immediately for this inadvertent, careless mistake. We are sorry this happened and will have it removed from any future DVD production." The head has now been edited to destroy any evidence of Bush, but the image lives on... Much to HBO's annoyance.
