At the start of the Old Man Logan storyline, we know that Logan is choosing not to fight the Hulks because of something that happened in his past. Later on, we learn that he hasn't popped his claws since the night the villains attacked and the X-Men were all wiped out, leaving Logan as the sole survivor.

It's only much later that we learn what truly happened that night to break Wolverine's spirit. The villains did attack, and Wolvie went into one of his berserker rages, slashing at all he saw and slaughtering those who were trying to hurt his teammates. Except that wasn't what happened at all.

"Did you really think you could do this alone? Take down forty super-villains? Talk about delusions of grandeur. But your friends. People who would hesitate. That's a different matter entirely.

Mysterio, the master of illusions, had tricked even Logan's enhanced senses to make him see his allies as the attacking enemies. Not only that, but the master of illusions even managed to make them feel and smell different. And, as they begged him to stop, Wolverine stabbed each and every one of his friends through the heart.
