10. Captain America: The First Avenger - 154
Steve Rogers once said that he didn't want to kill anybody, but after joining the army, he really didn't have a choice. The biggest death toll in his first movie came with his rescue of Bucky and the rest of the captured troops, as over 40 men, both US and HYDRA lost their lives.
Even more died during Cap's final attack on Schmidt's base, where more than 75 were killed. Adding in the two Norweigans at the start of the movie, those that were killed as Heinz Kruger attempted to steal the super soldier serum, the officers Schmidt shot when they questioned his work, the handful killed during the montage of Cap's missions and the three HYDRA soldiers he and Bucky killed on the train, The First Avenger saw over 150 people die.
Despite this however, arguably the two biggest deaths in the movie were later revealed to be fake outs. Bucky returned three years later in The Winter Soldier, and the Red Skull returned in Infinity War on Vormir as Thanos' guide to the Soul Stone.
This is all without the impossible to count number of comrades Bucky lost from the 107th after they were captured or killed in Azzano, or the condolence letters Col. Phillips was tired of dictating when Steve first asked about his best friend.
