Back before Doom and Wolfenstein got their respective reboots in recent years, People Can Fly were flying the "old school" first person shooter flag back in 2004.

The result of which is Painkiller, a frenetic shooter in the vain of old Doom, Quake and Serious Sam. It was loud, over the top and just sheer insanity to play.

But what are shooters of old synonymous with?

That's right, increasingly punishing difficulty levels, with Painkiller being no different.

The standard ending is very Doom: our hero Daniel is left to fight endless hordes in Hell. So far, so... normal for Painkiller's stor.

However, if you have the absolute stones to finish the game on Trauma, Painkiller's hardest difficulty setting, you'll be rewarded with an alternate ending.

It's the happiest of endings you could hope to get in this game, which is ironic as there is no happiness to playing it on Trauma.

Of course, I'm sure there are people that'll tell me it's easy and they've done it a billion times. But for the average, run-of-the-mill player, Trauma is going to be, well... traumatic.
