2. KillaVolt - Borderlands 3

If you played Borderlands 3 on release, then KillaVolt (aka Kenneth) is probably most memorable for one thing in particular: an attack that electrified the entire arena floor. The ENTIRE floor. It was unavoidable and as a player your only real option was to keep jumping and hope for the best.

It left a lot of us scratching our heads, convinced we must have missed something. There's no Gearbox gave this guy a sustained, unavoidable attack. He already had a shield that blocked all damage from the front and regular minion spawns to harass and distract you, who rarely dropped health.

The only way to recover after the electric floor was through luck, hoping he would spawn other enemies and then hoping those other enemies would drop health - if they didn't kill you first.

And we were right, as developer Gearbox eventually acknowledged in the patch that nerfed him that this was unintentional. So not only was KillaVolt "broken" in the sense of being a cheap jerk - he literally was broken.
