The Mass Effect trilogy is full of character deaths. After all, you lose Jenkins on the very first mission and there's nothing you can do to stop it. But most of the character deaths in the trilogy actually happen due to your choices. Sometimes it's obvious, such as when you have to choose to save either Kaiden and Ashley, whereas others such as Grunt's death aren't as clear cut in how you save them.
