On your travels you come across a village that has been razed to the ground, with the only survivors being a family of four. Two guards and a priest stand in judgement over them. When you approach, you discover that the village had been tormented by Malaria, and they had to resort to destroying it and killing everyone that was sick to prevent it from spreading.
From the pleading family with two small children to the implied threats by the priest, every interaction in this scene sets up saving the family as being the right choice. How could such nice people possibly harm anyone? If you choose to side with the family, they will flee and you will be forced to kill the guards, and you can leave the burned village feeling like a very good Samaritan.
Allowing the family to be killed turns out to not be as evil as it once seemed.
After they flee, they end up spreading Malaria to the entire island of Kephallonia, possibly damning hundreds of people to a slow and painful death. In what is a contorted adage to Philippa Foot's trolley problem, it seems the 'evil' choice was actually allowing the family to live.
