Compared to the other entries on this list, Pyramid Head spends significantly less time actively stalking the player, but to not include him would be a genuine crime.
Despite his appearances being more sporadic in nature, that doesn't stop him from being one of the most intimidating foes to ever grace the medium. From his initial appearance staring at the player from behind bars to his horrifying sexual assault of other monsters, each and every one of his appearances throughout the game is iconic in its own way.
Also unlike a lot of the other beasties on this list, you're forced into more than one boss fight against him, the first of which traps you in a confined space where you must simply avoid his sweeping strikes whilst running down the clock, the second has you fighting two Pyramid Heads armed with spears.
Neither of which are ideal scenarios, let me tell you.
His sudden appearance within a confined corridor at the game's mid-point is genuinely terrifying as the winding corridors leave very little room for escape, forcing you into a white-knuckle chase that never fails to bring the spookems.
