Incitatus is not so much an item as he is a horse. Or rather, a talking horse. Actually, it's more like an immortal, talking horse with godlike powers.

Despite being a historical strategy game based in the Middle Ages, Crusader Kings 2 is full of crazy events featuring the supernatural. One allows you to become immortal, and comes with unexpected attention from a certain equine figure.
Around two hundred years into your immortality, a horse god named Incitatus will send you a threatening message saying there can only be one true immortal, and it's not going to be you.
The horse wants to fight to the death, and while you can certainly oblige him, there is another, more peaceful way.
If you have a high enough learning stat, you can instead choose to engage Incitatus in a philosophical debate. This allows you to convince Incitatus that two immortals are better than one and that perhaps the horse should join your court.
Given Incitatus' incredible stats and the fact that he's immortal, this is the best deal you can get. Just choose your words carefully, as you only get to meet Incitatus once per campaign.
