Venture to the rat-riddled tower on Fyke Isle and you can use the lantern to watch conversations between spirits in their final moments to give you clues as to what has gone down here.

You eventually meet Anabelle who tells you a mage had been experimenting on rats, essentially giving them the Bubonic plague.

Peasants revolted and stormed the tower intending to do a lot of murdering so Anabelle took a potion from the mage assuming it would kill her peacefully but instead it simply put her into a deathless paralysis. The peasants were fooled into thinking she killed herself, as was her lover Graham who left her there, but when she awoke she was still unable to move and the plague-ridden rats slowly ate her alive. This has turned her into a Plague Maiden which honestly, fair enough, and you can help break the curse or let her and the plague loose on the mainland.
