Not so much a moment as an entire gameplay mode, Bioshock Infinite's unlockable 1999 Mode is basically the Nightmare version of the game. It is strictly for hardcore players, and as anyone who has given it a go knows, it makes the instantly accessible difficulties look like Pee Wee's Playhouse. Basically, everything that could possibly help the player, such as ammo, health, respawns, the navigation arrow and auto-aim, has either been heavily reduced or removed completely, while anything that already made the game more difficult, like the health of an enemy for instance, has been raised substantially. I never managed to get through more than an hour or so of 1999 Mode, but I did soon enough realise that it was quicker to flee from as many enemies as possible rather than stand your ground. The scarcity of ammo, the financial penalty for dying and greater toughness of enemies requires players to be super-smart in picking their engagements. Basically, you need to be a masochist to try and get through this, or have no money to buy any other games.
