3. Sacking The Director - Metal Gear Solid V: The Phantom Pain
Considering how shrouded in secrecy the whole Konami and Kojima fall out was (outside of a few key reports that highlighted how much the publisher was cracking down on supporters of the developer), we can never be sure just what was and what wasn't supposed to be included in the final version of Metal Gear Solid V.
That said, we can take a pretty solid educated guess. For instance, it's probably fair to say that Chapter 2 being made up primarily of re-done Chapter 1 levels wasn't the plan, nor was cutting the final mission.
Though one more mission wouldn't have saved the whole story, cutting it did leave more threads hanging than most would have liked. Likewise, kicking Kojima off the project a year out and pushing the game out with no delays very clearly resulted in a rushed final act that definitely hampered the impact of the sequel's biggest twist.
I can't say for certain had these elements not been dropped that MGS V would have lived up to the promise of being the final entry in this series, but it at least would have been given a fighting chance.
