One of the main environmental effects of the Death Stranding is the 'timefall', an acid-rain like element that has one major effect- rapidly speeding up time.

This can be seen throughout the trailers, such as the unfortunate Bridges member who is completely aged after heavy contact with the timefall. It's even seen effecting the wildlife, as flowers and grass grow and wither within seconds during the rain.

Although it's not really known how this will tie into gameplay, it's likely it'll act as a status decrease if you aren't sufficiently protected.

There also seems to be more than one way to combat the timefall. Fragile is seen eating a worm-like creature called a cryptobite that "keeps the timefall away". Whether these will be a consumable item ingame is unknown, but it seems like it prevents aging in some way.

Sam and other members of Bridges are also seen wearing protective clothing during the rain which means that it's likely that you can stay safe by wearing certain outfits.
