A psychiatrist's office is meant to be a safe space. A place of privacy, contemplation or at the very least, advice from someone who charges a lot of money for something you could probably read online.
And to be fair, Silvio Caruso won't have anything left to worry about after his encounter with 47. Stick that in your literary analysis.
The preparation behind this one makes it all the sweeter when Caruso is in the palm of 47's hand, lying there with no idea as to what he's in for. You could kill him prior in some of the steps, but why spoil it for yourself?
Funnily enough, listening to Silvio explain his actions leading to the mission is somewhat tragic, almost making you feel sorry for him. But of course, we're not here for a shoulder to cry on. That's what real psychiatrists are for.
No, 47 has a job to do. One that can be resolved with a pillow, some strong arms and a large amount of downward force onto the man's face. Much like fugu poisoning, the thought of having the life smothered out of you is a nasty one indeed.
