To delve into spoilers, the 46th mission, Truth, reveals that you're Big Boss in name only. Instead, you've been playing as the medic from the crash at the close of Ground Zeroes, albeit with facial reconstruction surgery making you the perfect stand-in for the real Snake. As such, if you look to either the side of the ACC during the evening - or get close to the glass exterior of the chopper whilst on Mother Base - you'll see your created character from the very beginning of the game staring back at you. This element of duality plays into the final reveal as Venom stares at his own reflection, listening to the words of Big Boss describe his role from here on out.
